What belongs in this log
A useful entry identifies the affected page or record, the previous state, the new state, the evidence or reason, the publication date, and whether a reader correction triggered the change.
- a project onion-address change;
- a material correction to history, terminology, or technical guidance;
- a directory record added, removed, or moved to history;
- a change to the verification or editorial methodology;
- a major WordPress, canonical, sitemap, or indexing change that affects public access;
- a policy revision that changes how readers or submissions are handled.
What does not belong
Do not create an update solely to refresh a date, manufacture a freshness signal, or announce a cosmetic edit. Small spelling and formatting corrections may be handled through ordinary revision history unless they change meaning.
How dates are used
The published date identifies the update entry. A page’s last-reviewed date changes only when its sources and conclusions have been substantively checked. The change log should make that work visible without pretending that every page was reviewed every day.
